One press will give league a red border and forces it into a mode where your mouse is supposed to be able to leave the window, pressing it a 2nd time disables this mode and locks your mouse Try putting the game in full screen and then back to borderless windowed and check if it changes anything. When I play with full-screen settings, there is a very clear offset of where my mouse cursor is located on screen, and where the game registers the click in-game, specifically, I have to hover my cursor significantly south and east of where I intend the click to go in-game.
